1.  I would NEVER fail to report my injury to my supervisor immediately

2.  I would NEVER go to a doctor chosen for me by the employer/insurance company for treatment

3.  I would NEVER allow a nurse case manager hired by the insurance company to be involved in my medical treatment

4.  I would NEVER allow the insurance company to fail to pay my mileage for driving to medical appointments
