- Report the injury to your employer

Under Iowa’s Workers’ compensation laws, your employer (or their insurance provider) has the right to choose the initial treating physician. - Employer/Insurance-directed providers
This is a doctor or healthcare provider chosen by your employer or insurance provider. Using healthcare providers on your insurance/employer’s network raises various concerns including:
- The insurance-appointed doctor controls your diagnosis and treatment plan. They may underestimate the severity of your injury. Their treatment recommendations may prioritize cost-saving over care. It is not uncommon for workers to feel rushed or dismissed during appointments.
- You may have limited access to specialists or second opinions
- Requesting a change of doctor
If you are unsatisfied with the employer or insurance appointed doctor, you can request a change of doctor. However, your employer or insurer must approve this change. If they fail to do so, you will need to file a petition with the Iowa Workers’ Compensation Commissioner to request the change.
- Seeking a second option (IME)
If there is a disagreement about your diagnosis, treatment or level of disability, you or the insurer may request an independent medical exam (IME). This, however, is not for treatment. An IME is a one-time evaluation by an independent doctor. The doctor is chosen and paid by the insurance provider. The IME can be requested by the employer/insurer or the claimant (injured worker).
The IME may be used to:
- Challenge the treating physician’s opinion
- Challenge your impairment rating when you’ve reached maximum medical improvement
- Influence whether your claim should be approved, denied or reduced
- Force you to return to work despite not having fully recovered
